Privacy Policy
I am committed to your confidentiality and privacy. In clinical practice every thing that occurs within the treatment space is confidential, unless I have medical or legal cause to act differently.
As part of my online booking system I take your name, telephone number and and email address. The booking system automatically sends reminders to both your phone and email. These details are held in a password protected server and are accessed only by myself and the administrative team at The Family Practice. If you would like to remove your details at any time from this system please contact me directly. If you have not used the system for 24 months, you will automatically be removed.
I also hold your name, date of birth, address, telephone number and email address in your clinical notes. These are held in a locked cabinet and are not shared with any third parties without your prior consent (if you are seeing another practitioner within the practice we may confer with your consent). I will keep these files for 10 years, in line with my professional responsibilty to data keeping. If you ever want to see the information I have stored, you can ask me for that at any time.
On occasion, I will send an email to give information about up-coming events, promotions, changes to my availability or a news letter. I will never use my client data-base to promote any third party events or information. I will contact you via email to ensure you are happy to receive this additional information, if, at any stage, you wish to opt out you can do so by sending me an email with "unsubscribe" in the subject box. This will only remove you from the additional mailing list, you will continue to receive reminders for your appointments.
And finally, if you ever think that there has been any error or omission in the treatment of your personal data, you have the right to complain to the ICO.
